Provider Kristina Samson PA-C is offering a limited number of spots for her Sage Health Direct program. This program was created to help those without insurance, but still desire regular healthcare visits. Please note, however, we still recommend having at least a catastrophic insurance plan as healthcare costs can be very expensive if needing emergency care, imaging or hospitalization.
Sage Health Direct is a one-year membership that includes:
- One annual physical with labs
- Labs include CBC, CMP, lipid, TSH, and A1C
- This is a common annual panel with well visits
- Additional labs available at reduced pricing
- Up to 5 additional visits. These visits may include:
- Sick visit (such as sore throat, cough, UTI, etc)
- Chronic care management (such as high blood pressure, diabetes, ADHD medication, etc)
- After hours telehealth visits available for urgent needs
- Guaranteed visit within 72 hours (unless provider is out of office that week)
The one-year membership cost is $999 or you can pay $85 monthly. At the end of one year, members will be asked if they want to renew although there is no guarantee the program will be offered every year. There are no refunds.
